Personal Health Record (PHR}?

PHR is an online resource to check your personal health record such as labs, immunizations, and 

medications.

Please see Chwa'kin Latham for the following;

• If you already have an account but don't remember your username or password.
• If you don't have an account and would like to register.
• If you would like more information on how the PHR website works.
