Patient Benefits
Personnel are available during business hours to assist with appointments, registration, SSI applications, Medi-Cal applications, Indian Health Services registration, and several other programs. Their office is located inside the main lobby or the Patient Benefits Coordinator can be reached at (530) 625-4261, Ext. 0325.
Appointments

What to bring to a clinic visit
- Tribal ID card
- Insurance cards: Medicare/Medi-Cal or Private
- Past medical records
- All medication you are currently taking
- Diabetic patient please bring your Glucometer
- Birth Certificate
The time to arrive for an appointment
- First visit – Plan to arrive 30 minutes before scheduled appointment time to permit the office personnel to register and to prepare your records for the medical provider and staff.
- Subsequent visits: Plan to arrive 15 minutes before scheduled appointment time to permit the office personnel to update your registration and/or billing information as necessary.
Late Arrivals
If you are unavoidably delayed for less than 10 minutes, an attempt will be made to work you into the schedule if possible. If your condition or the nature of your complaint is not urgent, you may be rescheduled to another day.

Missed Appointments
Keeping track of appointments is the patient’s responsibility. Patients who are on time are seen first and late patients will be on standby until an opening is available. It is the patient’s responsibility to call and make arrangements with the transportation department. If you need to be reminded of an appointment made weeks in advance, please request the appointment staff to remind you a few days before the scheduled appointment.
Tele-medicine Appointments
The following is the guidelines for your tele-medicine appointments: Be 15 minutes early for paperwork, consents, and vitals. You may not be seen if you arrive after your appointment time due to the specialist’s schedule. If you have three no shows you may not be able to be seen via tele-medicine due to UC Davis policies. All telemedicine appointments require a 4-day cancelation notice. Canceling with less than 4 days’ notice will count as a no-show. Please contact KMC if you have any further questions about your tele-medicine appointment.
New Patient Registration Paperwork
We invite you to complete your KMC New Patient Paperwork before you arrive for your scheduled appointment.
Once you have completed your New Patient Registration paperwork, you are able to submit the forms here.
You may also bring them with you to submit to our Patient Benefits Clerk on the day of your appointment.
