SUMMARY: The Certified Diabetes Educator Registered Nurse is responsible for the planning, coordination, and management of a comprehensive KMC Clinic Diabetes Program. This role provides culturally appropriate diabetes education, care coordination, and clinical support to improve diabetes prevention, management, and outcomes for tribal community members. The RN serves as a clinical leader, educator, and advocate, working collaboratively with medical providers, community health representatives, and other interdisciplinary team members.
JOB SPECIFIC SKILLS & ABILITIES:
- Provide comprehensive diabetes self-management education and support (DSMES) to patients and families across the lifespan
- Manage day-to-day operations of the Diabetes Program in alignment with tribal goals, grant requirements, and evidence-based standards of care.
- Develop, implement, and evaluate diabetes prevention and management initiatives, including individual and group education programs.
- Perform clinical assessments related to diabetes care, including glucose monitoring, medication education, lifestyle counseling, and risk reduction.
- Educate patients on nutrition, physical activity, medications (including insulin), glucose monitoring, and complication prevention.
- Collect, track, and analyze program data (A1C levels, participation, outcomes) for reporting and quality improvement.
- Assist with grant reporting, program planning, and sustainability efforts as required.
- Support prevention and management of diabetes-related complications (foot care, eye care, renal health, cardiovascular risk).
- Treats all information and data within the scope of the position with complete confidentiality and security.

- Participate in community outreach events, health fairs, screenings, and wellness activities related to diabetes prevention.
- Serve as a subject matter expert on diabetes care for clinic staff.
- Knowledge of diabetes standards of care and chronic disease management principles.
- Experience working with or willingness to learn about tribal communities and Indian health systems.
- Ensure diabetes education services meet national standards (ADCES/ADA/IHS guidelines).
- Work effectively within an interdisciplinary team and promotes a team approach to client care.
- Timely and thorough completion of client notes, documentation, and files.
- Assist with referrals, follow-up, and care transitions for patients with diabetes or prediabetes.
- Collaborate with primary care providers, pharmacists, dietitians, behavioral health, and specialty services to support coordinated diabetes care.
- Knowledge of trauma responsive care, addiction, recovery principals, and substance use disorder.
- Comfort with computerized documentation systems (Word, Excel, web-based applications and e-mail accounts)
- Willingness to be flexible with work schedule.
- Ability to perform a variety of duties dealing with people, often changing from one task to another without loss of efficiency or composer.

EDUCATION and/or EXPERIENCE:
- Education: Background in social work, AOD, health, nursing, psychology, or related field. Must have high school diploma and/or GED. Advanced degrees and/or certifications given preference.
- Licensure/Certification: Registered Nurse. Must be licensed in California and a graduate of an accredited school.
- Core Industry Standard:
- At least 2 years of professional RN practice after licensure.
- Minimum – 1000 hours providing diabetes care/education within the last 4-5 years.
- At least – 200 of those hours in the most recent year.
- 15 hours of diabetes-related continuing education before applying.
- LANGUAGE SKILLS: Ability to read and comprehend complex instructions, short correspondence, and memos. Ability to write business correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

CONDITIONS OF EMPLOYMENT:
Employment with K’ima:w Medical Center in contingent upon adherence to all applicable policies and regulations, including:
