SUMMARY: The CHR Navigator position is funded through First 5 Humboldt County in Collaboration with the Office of Child Abuse Prevention’s Road to Resilience Program. The CHR Navigator provides Supportive services to those who are pregnant and/or parenting and are currently or at risk of being impacted by substance use disorder. The Navigator will be responsible for providing services with a goal of providing health education, safety and injury prevention education and preventative health programs. The Navigator will collaborate with community resource leaders, physicians, and other members of the health care team cultivating relationships and serving as a community advocate for the needs of perinatal patients. The Navigator will be responsible for adhering to Mandated Reporting Responsibilities, as well as providing concise and accurate program data in order to maintain compliance with Road to Resilience Grant reporting responsibilities.
JOB SPECIFIC SKILLS & ABILITIES:
1. Serves as point of care contact for local providers to direct the patient to the appropriate resource.
2. Helps with coordinating multiple appointments, utilizing various electronic medical records and scheduling systems to ensure thorough communication across multiple disciplines.
3. Treats all information and data within the scope of the position with complete confidentiality and security.
4. A proven commitment and ability to professionally engage participants who are experiencing very stressful life situations/events in an empathetic, compassionate, and non-judgmental manner.
5. Works independently and is self-motivated to meet with and assist clients in resolving complex care issues.
6. Work effectively within an interdisciplinary team and promotes a team approach to client care.
7. Timely and thorough completion of client notes, documentation, and files.
8. Works closely with all departments in the clinic; attends and contributes to multiple case staffing meetings interacting with nurses, therapists, and other providers to share resources and provide services to families.
9. Act as an advocate on patients’ behalf.
10. Attend training as needed to assist with compliance and quality initiatives.
11. Ability to efficiently communicate with medical staff, patients, and co-workers.
12. Ability to ensure patient confidence and comfort.
13. Ability to read and comprehend written instructions, correspondence, memorandums, medical records, and scheduling forms.
14. Knowledge of trauma responsive care, addiction, recovery principals, and substance use disorder.
15. Culturally competent, knowledgeable of local Tribes, and their services.
16. Demonstrate initiative and ability to make independent judgments.
17. Comfort with computerized documentation systems (Word, Excel, web-based applications and e-mail accounts)
18. Willingness to be flexible with work schedule.
19. Ability to perform a variety of duties dealing with people, often changing from one task to another without loss of efficiency or composer.
20. Performs other duties as assigned.
EDUCATION and/or EXPERIENCE:
Education:
Must have high school diploma and/or GED.
Background in social work, AOD, health, nursing, psychology, or related field
Advanced degrees and/or certifications given preference.
