SUMMARY: Community Health Representative (CHR) provides supportive services to
persons in need of short- or long-term health care assistance. The CHR performs
routine health screenings in the community and in homes, health education, safety/
injury prevention education and preventive health programs.
FUNCTIONS & RESPONSIBILITIES:
1. Provide routine health screening consisting of blood pressure, pulse & respiration,
blood glucose & cholesterol monitoring.
2. Provide individual and community education addressing chronic disease, prenatal,
early childhood development, injury prevention and other identified topics.
3. Assist in case management and coordinate efforts with the Public Health
Nurse/Outreach Nurse.
4. Assist in providing health promotion and disease prevention programs in the
community.
5. Assist community members in complying with their health treatment plan.
6. Provide transportation services for clients of KMC, as needed.
7. Provides documentation in EHR of all patient contacts, i.e., delivery of medicine,
patient telephone calls, and delivery of patient supplies.
8. Documents findings in EHR and communicates abnormal findings to PHN, Outreach
Nurse, Director of Nursing and/or patient's provider per policy and procedure for the
Outreach Department.
9. Participate in public relations and in-service activities.
1 a.Attend and successfully complete all job-related trainings.
11. Occasionally accompany patients to their scheduled medical appointments and
assist with interpreting for patients when necessary.
12.Attend monthly staff meetings.
13. Provide supervisor with a schedule of planned field activities.
14. Confidentiality maintained in accordance with HIPPA laws and maintains boundaries
between staff and clients.
15. Builds trusting partnerships between clients and families.
16. Applies principals of strength-based approach in assisting clients and families.
17. Applies a Trauma Informed Approach in assisting clients and families.
18. Demonstrates cultural competency while working in the community.
19. Develops relationships with co-workers and community agencies to readily access
and share resources for family support.
20. Networks with other resources such as: Public Health Department, Northern
California Indian Development Council, Tribal Partner, California Rural Indian Health
Board, and others.
21. Uses observation skills, knowledge, and training to gather information about family
member to develop a plan of care.
22. Time management skills.
23. Perform other duties as assigned
JOB SPECIFIC SKILLS & ABILITIES
• AGE-Specific Competence: Must demonstrate the knowledge & skills necessary
to: 1) provide care appropriate to any age-related needs of the patients; 2) of the
principles of growth and development appropriate; 3) to assess and interpret data
about the patient's status to identify each patient's needs & provide the
appropriate care needed by KMC's Life Stage Patient Groups. Life Stage Patient
Groups are defined as infants, toddlers, preschool children, school age children,
adolescents, young adults, middle-aged adults, and late-stage older adults.
• Demonstrated ability to prioritize responsibilities.
• Proficient in Medical Terminology.
• Knowledge of Hoopa community, cultural awareness, and sensitivity.
• Demonstrated ability to perform all CHR charting responsibilities.
• Demonstrated ability to perform all functions and responsibilities as stated above.
EDUCATION and/or EXPERIENCE
- High School Diploma or equivalent (GED);
- one-year prior experience in a health setting
such as in a hospital, long term care facility or home health agency preferred. - Experience as a Community Health Worker (CHOW), Certified Nurse's Aide, Certified
Medical Assistant and or Health Educator preferred.
