SUMMARY: The Nurse Navigator position is funded through First 5 Humboldt County in Collaboration with the Office of Child Abuse Prevention’s Road to Resilience Program.   The Nurse Navigator provides case management services to those who are pregnant and/or parenting and are currently or at risk of being impacted by substance use disorder. The Nurse Navigator will be responsible for providing case management services with a goal of linking families to community-based agencies, resources for parenting and concrete supports to mitigate risks of substance exposure. The Nurse Navigator will serve as a link between healthcare providers and treatment settings including outpatient care, inpatient care, MAT/SUD treatment programs and will connect patients and families to resources for management of care needs. The Nurse Navigator will collaborate with community resource leaders, physicians, and other members of the health care team cultivating relationships and serving as a community advocate for the needs of perinatal patients. The Nurse Navigator will be responsible for adhering to Mandated Reporting Responsibilities, as well as providing concise and accurate program data in order to maintain compliance with Road to Resilience Grant reporting responsibilities.

SUPERVISORY RESPONSIBILITIES: This job has no supervisory responsibilities.

JOB SPECIFIC SKILLS AND ABILITIES:

  1. Serves as primary point of care contact for local providers to direct the patient to the appropriate resource.
  2. Offers assistance with coordinating multiple appointments, utilizing various electronic medical records and scheduling systems to ensure thorough communication across multiple disciplines.
  3. Treats all information and data within the scope of the position with complete confidentiality and security.
  4. A proven commitment and ability to professionally engage participants who are experiencing very stressful life situations/events in an empathetic, compassionate, and non-judgmental manner.
  5. Works independently and is self-motivated to meet with and assist clients in resolving complex care issues.
  6. Work effectively within an interdisciplinary team and promotes a team approach to client care.
  7. Timely and thorough completion of client notes, documentation, and files.
  8. Works closely with all departments in the clinic; attends and contributes to multiple case staffing meetings interacting with public health nurses, therapists, and other providers to share resources and provide services to families.
  9. Act as an advocate on patients’ behalf.
  10. Attend trainings as needed to assist with compliance and quality initiatives.
  11. Ability to efficiently communicate with medical staff, patients, and co-workers.
  12. Ability to ensure patient confidence and comfort.
  13. Ability to read and comprehend written instructions; able to interpret medical orders, correspondence, memorandums, medical records, and scheduling forms.
  14. Knowledge of trauma responsive care, addiction, recovery principals, substance use disorder.
  15. Culturally competent, knowledgeable of local Tribes, and their services.
  16. Demonstrate initiative and ability to make independent judgments.
  17. Comfort with computerized documentation systems (Word, Excel, web-based applications and e-mail accounts)
  18. Willingness to be flexible with work schedule.
  19. Ability to perform a variety of duties dealing with people, often changing from one task to another without loss of efficiency or composer.
  20. Performs other duties as assigned.

EDUCATION & EXPERIENCE:

  • Education:  Background in social work, AOD, health, nursing, psychology, or related field. Must have high school diploma and/or GED.  Advanced degrees and/or certifications given preference.
  • Licensure/Certification: Registered Nurse or Licensed Vocational Nurse. Must be licensed in California and a graduate of an accredited school.
  • Experience:  Payment based on experience.

Additional Courses required to complete during the first year to 18 months include:

4) California Comprehensive Perinatal Service Program Orientation and HIS Injury Prevention Program all levels.


CONDITIONS OF EMPLOYMENT: 

Employment with K’ima:w Medical Center in contingent upon adherence to all applicable   policies and regulations, including:

  • Compliance with Title 21, the Hoopa Valley Tribe’s Drug and Alcohol Policy, and the Drug Free Workplace Act.

  • Successful Completion of Title 30A Background Check process.

  • Compliance with Title 13 the Hoopa Valley Tribe’s Employment Rights Ordinance.

  • Must have a valid California Driver’s license, and insurable.
